چکیده

hypotension occurs during spinal anesthesia. Only one prior case has been published describing profound hypotension in a parturient with bipolar disease, controlled with lithium and risperidone, undergoing spinal anesthesia for Cesarean delivery. The patient was refractory to conventional treatment with ephedrine and iv fluids, and eventually responded to large doses of phenylephrine.2 The treatment should consist of iv fluids and sympathomimetic drugs. Epinephrine and dopamine are not recommended, as ß-stimulation may worsen hypotension due to risperidone-induced α-adrenergic antagonism.

Utility of Intraoperative Neuromonitoring during Minimally Invasive Fusion of the Sacroiliac Joint

Study Design. Retrospective case series. Objective. To document the clinical utility of intraoperative neuromonitoring during minimally invasive surgical sacroiliac joint fusion for patients diagnosed with sacroiliac joint dysfunction (as a direct result of sacroiliac joint disruptions or degenerative sacroiliitis) and determine stimulated electromyography thresholds reflective of favorable imp...

Reply to letter to the editor regarding cardiac arrest after vagal stimulation in intraoperative neuromonitoring.

To the Editor: We are grateful for the interest in our article “Cardiac arrest with vagal stimulation during intraoperative nerve monitoring” and appreciate the comments by Dr. Sclabassi et al. First, we would like to apologize for erroneously referring to the current of 1 mA as a voltage. Regarding stimulus width and pulse frequency, our default values are: stimulus width, 0.1 mS, and frequenc...

[Intraoperative neuromonitoring in thyroid surgery].

BACKGROUND Intraoperative neuromonitoring of the recurrent laryngeal nerve in thyroid surgery facilitates the identification of anatomical structures in cervical endocrine surgery reducing the frequency of vocal cord paralysis. OBJECTIVE To study the normal electrophysiological values of the vague and recurrent laryngeal nerves before and after thyroid surgery. To compare rates of injury of r...

Identification alone versus intraoperative neuromonitoring of the recurrent laryngeal nerve during thyroid surgery: experience of 2034 consecutive patients

BACKGROUND The aim of this study was to evaluate the ability of intraoperative neuromonitoring in reducing the postoperative recurrent laryngeal nerve palsy rate by a comparison between patients submitted to thyroidectomy with intraoperative neuromonitoring and with routine identification alone. METHODS Between June 2007 and December 2012, 2034 consecutive patients underwent thyroidectomy by ...

Interpretation of intraoperative recurrent laryngeal nerve monitoring signals: The importance of a correct standardization.

INTRODUCTION Despite the increasingly broad use of intraoperative neuromonitoring, review of the literature and clinical experience confirms there is little uniformity in application of and results across different centers. The aim of this study was to evaluate the ability of intraoperative neuromonitoring with a standardized evaluation of the signals to predict the postoperative functional out...
